REGISTER FOR MINQUIZ
Registration for the quiz is open until 30th April 2018. However late registration are welcome if space is still available. Only three Grade 12 learners per school are allowed to register, and they may be accompanied by a chaperone or educator on the day of the quiz. All learners should have Mathematics (not Mathematical Literacy) and Physical Science as subjects. Registration for the provincial leg is free, but schools are required to provide their own transport. Click on the link below to register. Name of school Principal or Educator (Mathematics or Physical Science)
Name of Chaperone (if different from Principal or Educator)
Contact details of school/Educator or Chaperone (Tel no, cell no & e-mail address)
Names of 3 Grade 12 learners
School’s Grade 12 final average percentage for Mathematics, Physical Science and English in 2015, 2016 and 2017
Any dietary or special needs for the team and chaperone
